Job Description

Nurse Practitioner – Virtual Primary & Complex Care

Delaware | Full-Time | Primarily Remote | $135,000 Annually + Benefits

We are seeking an experienced Family Nurse Practitioner (FNP) or Adult/Adult-Gerontology Nurse Practitioner to provide longitudinal, primarily virtual care to medically complex adult patients throughout Delaware.

This opportunity is best suited for an experienced Nurse Practitioner with a strong and recent primary-care background, demonstrated experience managing patients longitudinally through telehealth, and direct exposure to value-based care.

The organization serves medically complex populations through an integrated, value-based care model focused on proactive clinical management, early intervention, improved quality outcomes, and reducing avoidable emergency department visits and hospitalizations.

Responsibilities

  1. Provide ongoing, longitudinal patient management through telehealth and remote-care technologies.
  2. Independently assess, diagnose, and treat acute and chronic medical conditions.
  3. Manage medications, laboratory and diagnostic testing, treatment plans, and ongoing follow-up.
  4. Provide primary and complex care to adult and geriatric patients with multiple chronic conditions.
  5. Develop and maintain individualized care plans based on patients' clinical needs and goals.
  6. Identify changes in patient condition and intervene early to reduce avoidable ED utilization and hospitalization.
  7. Coordinate care with physicians, specialists, caregivers, home-health providers, health plans, and interdisciplinary team members.
  8. Support patients through hospital admissions, discharge, and transitions back into the home.
  9. Perform medication reconciliation and address post-discharge clinical needs.
  10. Participate in advance-care planning and goals-of-care discussions.
  11. Identify and address clinical and preventive quality gaps.
  12. Participate in value-based quality initiatives, including HCC/risk adjustment, HEDIS/STAR measures, and appropriate diagnostic documentation.
  13. Contribute to clinical best practices within an evolving complex-care model.

Required Qualifications

  1. Master's degree or higher in Nursing.
  2. Current certification as an FNP, Adult NP, or Adult-Gerontology NP.
  3. 3+ years of Nurse Practitioner clinical experience, with 5+ years strongly preferred.
  4. Strong and recent NP-level primary-care experience managing adult and/or geriatric patients.
  5. Demonstrated experience providing longitudinal telehealth care, including managing the same patients over time rather than exclusively episodic visits or one-time assessments.
  6. Experience independently managing chronic and medically complex conditions.
  7. Experience working within a value-based or quality-driven healthcare model.
  8. Active Delaware NP/APRN licensure and appropriate prescriptive authority, or eligibility and willingness to obtain Delaware licensure.
  9. Strong clinical judgment and ability to practice independently within a primarily virtual environment.

Highly Preferred Experience

Candidates with direct experience in several of the following areas will receive strongest consideration:

  1. Medicaid and/or Dual-Eligible populations
  2. Medicare Advantage
  3. HCC coding and risk adjustment
  4. HEDIS and STAR measures
  5. Quality-gap identification and closure
  6. Population health
  7. Value-based reimbursement models
  8. Care transitions and post-discharge management
  9. Avoidable ED/hospitalization reduction
  10. Home-based complex care
  11. Geriatric care
  12. Advance-care planning
  13. Interdisciplinary care coordination

Candidates currently residing and/or practicing in Delaware are strongly preferred.

Ideal Candidate

The strongest candidate will be an experienced Nurse Practitioner who is currently or recently practicing primary care, has managed medically complex patients longitudinally through telehealth, and has direct experience working within a value-based care environment.

This is not primarily an episodic telehealth, urgent-care, health-risk-assessment, or chart-review position. The NP will establish ongoing clinical relationships and manage medically complex patients across the continuum of care.

Compensation & Schedule

Compensation: Approximately $135,000 annually

Schedule: Full-time, 40 hours per week

Work Model: Primarily virtual/remote

Benefits: Health insurance and additional employer-sponsored benefits
